Ways You Can Contribute to This Special Event

There will be a new section on the website for the upcoming Forum. We've set aside a place on the site, and also in the Program that will be handed to all Forum attendees, where we will list the names of those being honored. It's a place for you to remember a loved one who has passed and/or to honor someone special. View the contributors listing here.

The California PH Forum is made possible by the dedication of a small working committee, mostly made up of PAH patients, and by contributions from our corporate and individual sponsors. Here is a way you can help.

You may make a donation in any amount in memory of a PAH patient who has passed, or in honor of your caregiver, your doctor, a family member or other important individual or group in your life. This is a great opportunity to say "thank you" while helping a very worthy endeavor.

Checks may be made payable to California PH Forum and mailed along with the names of the donor/s and the person/s you wish to honor or remember to:

California PH Forum
PO Box 508
Danville, CA 94526

or

PAYPAL - Use PayPal and send an email with donation details to:
caphforum@sbcglobal.net

You may also send an email with donation details to info@caphforum.com. If you wish to make a substantial tax-deductible gift we will provide you with our tax identification number and your check will serve as your receipt.

Raffle gifts from individuals or businesses may be sent or brought to the Forum event. Contributions of your time and talents are also very much appreciated.  Email us with your comments.

Thank you for helping us to keep the Forum going strong!
